FORMS CHECKLIST
Please be sure you submit the following in order to travel with us on the NYC Theatre Trip scheduled for January 10-15, 2012.

Return EACH of these immediately to secure a spot on the trip:

• Contact Information Form
  Completed online by the traveler (Online Form). A paper form is also available (PDF Form).
• Agreement Form
  (Online Form / PDF Form)
  Signed by the traveler and by the person responsible for payment. Must be submitted with deposit payment OR Application to Charge Travel Expenses to FHU Student Account Form.
• Application to Charge Travel Expenses to FHU Student Account
  • Completed only by those enrolling in the Theatre Survey course in January AND are planning to charge trip costs to their account.
  • Completed by traveler and/or person responsible for payment.
  • Form must be initialed by an FHU Financial Aid officer.
• Payment and Cancellation Policy
  (Online Form / PDF Form)
  Signed by the person responsible for payment.

Return these BEFORE Thanksgiving:

• Trip Add-ons
  (Online Form / PDF Form)
  Completed by the traveler and/or person responsible for payment. Submit as soon as possible to secure your add-ons.
• Hold Harmless Form
  (Online Form / PDF Form)
  Signed by the traveler.
• Consent and Release for Medical Purposes
  Signed by the person responsible for medical expenses.
• Medical Information Form
  Completed by the person responsible for medical decisions.

A limited number of spots are available. The best way to secure your spot is to submit all paperwork no later than October 20, 2011. Preference will be given to those travelers who have made arrangements for payment and have submitted the completed forms.
