FERPA Privacy Release Form

Students need to update this information as necessary.

 

Declaration of Major Form

Use form if you have never declared.

 

Change of Major Form

Submit to the department of your previous major or Registrar Office. They will work with the appropriate staff in your new major to transfer your file.

 

Student Record Release

Form used to release any records from your student file i.e., immunization records, unofficial copy of ACT scores, etc.

 

Transcript Request

Electronic transcript delivery is available through Parchment Exchange.

 

Undergraduate Course Withdrawal Form or Graduate Course Withdrawal Form
